Notice of Sewage Event Claim Form

If you experience a sewage disposal system event you must file a written claim with the City of Eaton Rapids within 45 days after the date the property damage was discovered.  Please use the claim form above and return it to City Hall.

Complete the Automatic Payment Withdrawal form and return it to the Clerk’s Office to start you electronic payments. An authorized signature is required.
